
Horizons,
Inc. is in the process of developing and implementing a new program that will
provide transition care coordination services to consumers. Funding has been received from private
foundations and WCSN for this pilot project.
The program will provide temporary shelter (30-60 days) by utilizing 4
beds which are located in Horizons' residential facility. Temporary shelter is an immediate need of
women entering the program, but only one important aspect of the program. Each participant will have access to the
services offered through Horizons' residential and outpatient treatment
programs. These services, designed to
ensure sobriety and stability, include individual counseling, substance abuse
groups, vocational counseling, basic life skills training, and full access to
Horizons' residential Alcohol and Other Drug Abuse (AODA) programming. Additional services will target those areas
that pose the greatest risk to women participants following discharge and can
include education and assistance in the areas of housing, employment,
transportation, and childcare.
